What's the Best Next Step?

#34

A 32-year-old woman presents with a painless, firm nodule measuring 7 mm in her right upper eyelid that has been slowly enlarging over the past two months.

She is instructed to perform daily gentle but firm lid massages and apply warm compresses to help melt the viscous lipids within the obstructed meibomian gland.

After four weeks of strict compliance with this conservative regimen, the localized tarsal plate nodule remains completely unchanged in size and continues to cause a cosmetically unacceptable deformity.

Case Summary

Clinical Focus:

  • A 32-year-old female presents with a large, chronic upper eyelid chalazion that is completely refractory (unresponsive) to standard conservative management including thermal therapy and mechanical gland massage.

Diagnostic criteria and features of a chalazion include:

  • A firm, palpable, non-tender, and non-erythematous subcutaneous eyelid nodule.
  • Pathophysiology driven by the sterile obstruction of a sebaceous meibomian gland within the dense connective tissue of the tarsal plate.
  • Higher baseline frequency in the upper eyelid due to an increased length and volume of meibomian channels.
  • Secondary structural complications if large, including induction of mechanical ptosis or corneal distortion.

When conservative attempts to evacuate the blocked lipid material fail after an extended period, moving to localized structural intervention is the primary evidence-based standard of care.
