A 26-year-old man presents with severe, unilateral right-sided throat pain, high fever, and extreme difficulty swallowing both solids and liquids.

He demonstrates a muffled 'hot potato' voice, noticeable drooling due to severe odynophagia, and marked trismus that limits his maximum inter-incisor opening to less than 20 mm.

Upon dynamic oropharyngeal inspection, the right tonsil is severely swollen and displaced medially, forcing the palatine uvula to deviate sharply to the left side and partially occluding the structural pathway of his upper airway.

Case Summary

Clinical Focus:

A 26-year-old male presents with a severe peritonsillar abscess (quinsy), a critical acute Ear, Nose, and Throat (ENT) deep space neck infection causing significant trismus and upper airway narrowing.

Diagnostic criteria and red flags for a peritonsillar abscess include:

Severe unilateral sore throat accompanied by a hot potato voice and drooling.
Marked trismus resulting from inflammatory spasm of the internal pterygoid muscle.
Medial displacement of the affected tonsil and tonsillar pillar with a contralateral deviation of the uvula.
Potential tracking into deep fascial neck spaces, which can lead to life-threatening airway occlusion or vascular complications.

When an expanding deep neck abscess causes acute trismus, uvular deviation, and visible narrowing of the structural pharyngeal airway, immediate surgical drainage is the standard of care.
