Clinical Focus:

  • The diagnosis is Essential Hypertension (HTN).

Aetiology/Cause:

  • Multifactorial genetic predisposition interacting with environmental factors such as high dietary sodium intake and physical inactivity.
  • Idiopathic activation of the sympathetic nervous system and the renin-angiotensin-aldosterone system leading to increased total peripheral resistance.
  • Absence of an identifiable secondary cause of hypertension, which accounts for approximately 90% to 95% of all clinical presentations.

Clinical Features:

  • Typically asymptomatic in the early or moderate stages, often referred to clinically as a silent killer.
  • Sustained elevations in systemic arterial pressure with a systolic blood pressure greater than 130 mmHg or a diastolic blood pressure greater than 80 mmHg.
  • Absence of acute symptoms or target organ damage during initial staging examinations.
  • Gradual development of left ventricular hypertrophy, retinal microvascular changes, or mild proteinuria over years of unchecked elevation.
